Specialty Nurse – First Assist

Perioperative Services

The Registered Nurse First Assistant (RNFA) carries out functions that will assist the surgeon in performing a safe operation with optimal results for the patient. The RNFA practices perioperative nursing and has acquired the knowledge, skills and judgment necessary to assist the surgeon through organized instruction and supervised practice. The RNFA practices under the direct supervision of the surgeon. Serves patients from neonate, infant, pediatric, adolescent, adult and geriatric.

Essential Functions

  • Performs preoperative patient assessment and teaching.
  • Collects and analyzes data and assesses patient in developing nursing diagnosis.
  • Provides a safe environment for patient and staff by implementing standards of aseptic technique and by controlling the physical and psychological aspects of that environment.
  • Communicates effectively with the patient, staff, physicians, and families.
  • Maintains knowledge of equipment set-up and function.
  • Prepares for surgical cases in an efficient manner, anticipating needs of patient and surgeon, and provides creative strategies in meeting those needs.
  • Applies knowledge of surgical anatomy, physiology, and operative technique relative to operative procedures wherein the RNFA assists.
  • Provides hemostasis by clamping blood vessels, coagulating bleeding points, legating vessels and by other means as directed by the surgeon.
  • Provides exposure through appropriate use of instruments, retractors, suctioning and sponging techniques.
  • Handles tissue as directed by the surgeon during the operative procedure.
  • Performs wound closure as directed by the surgeon; sutures the fascia, subcutaneous tissue and skin.
  • Applies surgical dressings.
  • Assists with transferring the patient from the operating room.
  • Serves as a resource person to staff and physicians.
  • Functions in circulating or scrub role when not utilized as RNFA.

Marginal Functions

  • Participates in orientation and training of new personnel.
  • Assists in maintenance of physician's preference cards and other O.R. resource material.
  • Utilizes computer in disposition of specimens and obtaining pertinent information.
  • Supports and participates in departmental Performance Improvement projects.
  • Attends meetings and inservices.

Minimum Knowledge, Skills And Abilities Required

  • Possession of a current or compact state license as a registered nurse issued/defined by the State of Iowa.
  • BSN desired.
  • Basic Life Support certified within six (6) weeks of hire. The acceptable credentialing body is the American Heart Association.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
  • Basic EKG course encouraged.
  • Proficient in perioperative nursing practice both as a scrub nurse and circulating nurse.
  • Current CNOR (Certified Nurse in OR) or obtained with six months of completing RNFA training program.
  • Completion of an RN First Assistant course at an institution approved by the Association of Higher Learning, in which the curriculum addressed all of the content areas of the Core Curriculum for the RN First Assistant and a degree or certificate was awarded.
  • Current CRNFA (Certified Registered Nurse First Assistant) desired.
  • Ability to read, write and speak English language.

Physical Activity Requirements

(Constant 67-100% of workday, Frequent =34-66% of workday, Occasional = 33% or less of workday)

CONSTANT

  • Talking and hearing: to exchange information and ideas by means of spoken word, both by phone and in person.
  • Standing: while functioning in the scrub role.
  • Walking: usually at average speed, but occasionally faster, when obtaining supplies, etc.

FREQUENT

  • Lifting: while moving patients, supplies, equipment, etc.
  • Kneeling, bending, stooping, crouching: while performing perioperative duties (retrieving supplies, counting sponges, changing suction canisters, etc.)
  • Grasping: while handling surgical instruments.
  • Fingering: working primarily with fingers including sutures, needles, etc.
  • Pushing and pulling: while moving patients, equipment.
  • Climbing: while using stairway between locker area and the O.R. and between Inventory Control and the O.R.

Physical Demand Requirements

  • Heavy work: Exerting up to 100 pounds of force occasionally and/or up to 40 pounds offorce frequently, and/or up to 20 pounds of force constantly to move objects.

Visual Acuity Requirements

  • Preparing and reading written and electronic documentation
  • Working with small sutures, needles, etc.
  • Good hand-eye coordination.
  • Peripheral vision.
  • Ability to work with a microscope.
  • Subject to extremes in lighting.

  • Ability to maintain a high standard of courtesy and cooperation in dealing with co-workers, patients and visitors, and perform job functions satisfactorily despite the stress of a hospital work environment.

Equipment/Tools

  • Standard O.R. equipment including, but not limited to, all types of surgical instrumentation, cautery, lasers, microscopes, video systems, power equipment. Also use of phone, computer terminal and written materials.

Working Conditions

  • Colleagues in this job classification have been identified as having the likelihood of occupation exposure to blood or other potentially infectious materials and, therefore, are included in the OSHA Exposure Control Plan with all its specifications for preventing contact with the above materials.
  • The worker is subject to inside environmental conditions.
  • The worker is subject to noise from various types of surgical equipment
  • The worker is subject to vibration, (example: from drills, saws, etc.)
  • The worker is subject to hazards from electrical equipment, mechanical parts, lasers, etc.
  • The worker is subject to gases and fumes (example: bone cement).

  • Reports to Operations Manager.
